Enrollment Agreement
Emergency Information Form | Note that "Name of Person Authorized to Pick up Child (daily)" is a required field. If the child's parents are the ones who are authorized to pick them up daily, please just write "Parents" in this field. Additionally, please write "NONE" on the back side of the form if the questions do not apply to your child.

Health Inventory
Blood Lead Testing Certificate (Required only for children entering preschool, K, or 1st grade)
Immunization Certificate | Any completed and signed Immunization Form from your doctor's office is fine.

Release of Information Form

This form gives TPCDC staff permission to share information with an outside professional working with a child (e.g. a speech or occupational therapist.)
